Advance care planning (ACP) is the process of thinking about what matters most to people in life and what that means for health and personal care choices. ACP helps people share their wishes and preferences with the people who are important to them (Advanced Care Planning Canada, 2024). During this session, participants will learn about ACP and explore their role in supporting clients to engage in the process. Tools that can be used as part of ACP will be shared, along with real-life examples of how the material can be applied to day-to-day work. Participants should expect to have their cameras on, complete poll questions, be ready to share situations that they have experienced with clients, and participate in small group discussions.
